Hair Extensions Deposit *

Please check all the following after thoroughly reading each clause.

I understand that in order to book a hair extensions service I must pay a 100% non-refundable retainer fee for the hair extensions at the time of booking. Failure to do so will result in no appointment being made. Under no circumstances will the deposit for hair extensions ever be refunded.

I understand that once the appointment has been made and the hair has been ordered, and if I cancel, the hair that has been ordered will be surrendered to The Eclectic and I will have no ownership of the hair.

Hair History *

Please check all the following after thoroughly reading each clause.

I have made my stylist, Kia, aware of all the services and products I have used and/or I have done, or another stylist has used/done to my hair. I understand that withholding information can negatively affect the outcome of my hair extension service. I realize that this is extremely important information and if I withhold any information this may lead to damage to my hair and an unpredictable result in the process.

Hair Maintenance + Hair Care *

Please check all of the following after thoroughly reading each clause.

I have been made aware of the process I will need to go through during this service including maintenance in the following months. I understand that hair extensions can, and will, cause damage if I do not take care of my extensions as instructed.. I understand that I will need to use proper shampoo, conditioner, and hair treatments. Lack of using proper hair care or recommended hair care may result in my extensions slipping, matting, or damaging my own hair, and my extensions altogether.

I understand that proper hair care will help my hair extensions last longer and have better retention.

Hair Damage + Unauthorized Removal *

Please check all of the following after thoroughly reading each clause.

I also understand that this current hair service being performed by Kia Yang may also cause little to extreme damage to my hair. I have been made fully aware of the hair damage and hair loss that can ensue during and after this process and am still fully giving consent to continue with this service.

I understand that any damage done to my extensions after I have left the salon is completely within, and due to, my own control, and choices, and is of no fault to The Eclectic team.

Any hair extension that is forcibly removed by accident, or on purpose, is also no fault of The Eclectic team. In this circumstance all damage repair and re-application is a fully charged service. I understand that if I remove my own extensions then I am at fault for any damage caused to my own hair, or to the extensions.

Adjustments + Refunds *

Please check all of the following after thoroughly reading each clause.

Hair extensions retainers are non refundable and cannot be refunded under any circumstances.

An adjustment can be done but the salon must be notified of any dissatisfaction within 2 weeks after the initial service. Anything after 2 weeks is a full service charge.

A partial refund of 30% of the service total on the day of the appointment can be made after a stylist has been able to correct said error, and if the client is still unhappy after said correction.

The hair extensions itself can be exchanged back to the rightful hair vendor if the box/seal has never been broken per the vendor’s policies. Any hair extensions that have been removed from the box, been colored, and worn cannot be exchanged.
